What information belongs on a copyright page?

A complete copyright page (page iv, the back of the title page) usually carries:

• Copyright notice — © year and the rights holder's name.
• "All rights reserved" rights statement.
• ISBN(s) — one line per format you're publishing.
• Edition and printing information, if you use them.
• A disclaimer appropriate to your book type (fiction, memoir, nonfiction).
• Imprint name and location, if you have one.
• Credits — editor, cover designer, etc. (optional).
• LCCN and any legal notices, if applicable.
