Writing under a pen name? Copyright is usually held under your legal name or your company — put that here and keep the pen name above.
The “10 9 8 7 6 5 4 3 2 1” under the edition. Its lowest number is the printing number, struck off each time a book goes back to press — which never happens with print-on-demand, so leave it off unless you’re doing an offset run.
Sets a line like “Printed in the United States of America” near the foot of the copyright page. It never appears in your ebook — an ebook is not printed anywhere.
Your publishing name. Leave blank and “Published by” simply won’t appear.

Turn this off if your mark already says it. The name still appears on the copyright page and in your ebook’s details, and your location and website stay on the title page either way.
Sits at the foot of the title page, above the imprint line. A PNG with a transparent background prints best. Height is set in lines of type, so a large file cannot push the imprint off the page — give us the picture, not the size.
One per format, each labeled correctly. The two hardcovers are separate products and need separate ISBNs — fill in only the one you are printing. No ISBN yet? Come back and add it — everything stays in this browser.
A free number the Library of Congress hands out before you publish, so libraries can catalog your book. Apply at the Library’s PrePub Book Link — self-publishers get an account straight away. Taking a number commits you to mailing the Library a printed copy once the book is out.

The pages after “The End.” Only sections you fill in appear — each becomes its own page.
Everything else a book conventionally carries. Pick one and it lands where the convention puts it — a foreword before a preface, a preface before an introduction, a colophon last — so you never have to think about the order. A page you leave empty simply doesn’t print.
